1）MEDIA CONSENT RELEASE
As a guardian of my child/'s, I hereby irrevocably consent to allow BOM and their authorized designees, in their sole discretion, to take photographs/recordings of my child/'s, and to use such photographs/videos for educational and marketing purposes at any time and by any means of distribution/media. I hereby irrevocably waive any right students may have to compensate for uses and any right to inspect or approve any of the photographs and videos containing the Student's image or likeness.
To the maximum extent permitted by law, I agree to release and hold harmless BOM and their respective officers, directors/ trustees, agents, employees and affiliates from any claims, damages, liabilities, and/or losses that may arise from the uses set forth in this Agreement.
2）Release of Liability
I understand that there are risks of injury, illness, Covid-19, damage, loss and others risks involved with my or my child/’s participation in the BOM. Regardless of the cause of the above risks and the location where injury and illness happens, including but not limited to the locations of recitals, teaching, and events. I do hereby waive, release, absolve, indemnify and agree to hold harmless to BOM.
3) Dropping of/Picking up
It is parents or guardians responsibility in person to drop off students to classes/events/teachers picking up students from classes/events/teachers. I understand that there are risks of dropping off/picking up students in parking lots.
4)Cancellation
Cancel/Switch/Make up lessons: as lease a 24-hour notice, the lesson will be charged within a 24-hour notice.
Cancel Package Plan( Yearly package, Spring/Fall/Summer Package and more plans): The lessons which have been taken will be charged as a single lesson fee, refunding the rest fee from the plan.
All lessons of the package need to be completed within the package date, the lesson will be expired after the package date.
School Year Package: Sep to next May 31
Spring: Jan to May 31
Summer: June to Aug 31
Fall session Sep to Dec 20
I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, HAVE SIGNED IT ON BEHALF OF MYSELF AND STUDENT VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE, AND CERTIFY THE ACCEPTANCE OF ITS TERMS.

1)肖像授权
作为孩子/们的监护人,我在此不可撤销地同意BOM及其授权人员自行决定为我的孩子/孩子们拍照和/录制视频,并可随时使用此照片和视频,用于教育和营销目的,通过任何分发/媒体方式.我在此不可撤销地放弃起诉BOM使用学生照片和视频获利的权利。
2)责任豁免
我了解我或我的孩子（们）在参与BOM时存在受伤、疾病、Covid-19、损坏丢失物品和其他风险。 无论上述风险的原因是什么，本人特此免除对BOM的责任和赔偿责任。
3)亲自接送
家长或监护人有责任亲自将学生送到班级/活动/老师和从班级/活动/老师接学生。 我了解在停车场接送学生存在风险。
4)取消课程，取消套餐计划
取消课程/调课补课至少提前24小时通知,少于24小时通知照常扣费.
取消套餐: 套餐中的已上课程按单词课收费，退剩余课费。
注册课程需要在注册学期结束前完成。
学年套餐：9月到次年5月31日
春季套餐：1月到5月
夏季套餐：6月到8月31日
秋季套餐：9月到12月20日
我已阅读本协议，完全理解其条款，代表我自己和学生自愿签署，没有任何诱导、保证或保证，并证明接受其条款。
